Referral Details

By completing this referral form you agree to abide by these terms:  

Persons who refer potential clients to FPC in 2020 are eligible to receive as compensation, only one of two following options:

Monetary Compensation:

As consideration for referring an individual practitioner or health professional to FPC, you are eligible to receive $2,400.00, subject to these terms and conditions: FPC will pay you $1,200.00 only after the referred individual’s (“New Client[s]’”) first payment for the FPC Program. FPC will pay this amount by the end of the financial quarter in which the new client makes first payment. FPC will pay you $1,200.00 only after the new client has been enrolled in the FPC program for a period of one year and is current on their payment obligations under their contract. FPC will pay this amount by the end of the financial quarter in which the new client completes their first year. You must be current on your payments to be eligible receive any monetary amount as consideration for the New Client. If you are behind on payments, the monetary compensation for each referral will be deducted from your past due balance.  

FPC Concierge Site Visit:

As consideration for referring two practitioner or health professionals to FPC, you are eligible to request and receive an on-site training session (“OSTS”) from an agent of FPC (“Agent”), subject to these terms and conditions: The Agent hosting the OSTS and the subject of the OSTS shall be chosen at FPC’s sole discretion. The OSTS will be limited in duration to one session, six (6) hours in length, starting from the moment the Agent arrives at your practice or office, not including a one hour break for the Agent to eat lunch. You may not divide this time among multiple days. You are not eligible to receive or schedule an OSTS until the New Client has paid their first two full payments for the FPC Program. FPC will provide you with an OSTS within a period not to exceed one (1) year from the date you become eligible to schedule the visit. (FPC will make all reasonable efforts to provide the service at the earliest opportunity.) FPC will provide you with three calendar months in which to schedule your OSTS, (“OSTS Scheduling Period”). You must choose a date within that period. If you schedule no date within the OSTS Scheduling Period, you may forfeit your eligibility for an OSTS and may instead receive monetary consideration under Section (1)(a). You must be current on your payments to be eligible to receive any monetary amount or OSTS as consideration for the New Client. If you are behind on payments, the monetary compensation as set forth in Section I(a) for each referral will be deducted from your past due balance. If you refer a single practitioner or health professional and concurrently communicate to FPC that you would like opt for the concierge visit as opposed to monetary compensation, you will have one year from the date of your first referral to refer another practitioner or health professional. If you fail to refer another practitioner or health professional within that time period, you will automatically be opted into the monetary compensation plan laid out in Section I(a).
